Coronary computed tomography angiography (CCTA) plaque characteristics and hemodynamic parameters better predict future coronary events than clinical factors alone. High-risk plaque characteristics (HRPCs) add value to risk prediction in vessels without significant FFRCT.
Area of Science:
- Cardiovascular Imaging
- Interventional Cardiology
- Radiology
Background:
- Coronary computed tomography angiography (CCTA) assesses coronary artery disease (CAD) plaque but its prognostic role is unclear.
- Evaluating CCTA-derived anatomic and hemodynamic plaque characteristics can improve risk prediction for CAD events.
Purpose of the Study:
- To assess the prognostic implications of CCTA-derived anatomic and hemodynamic plaque characteristics in predicting subsequent coronary events.
- To compare the predictive abilities of CCTA-derived parameters versus clinical risk factors.
Main Methods:
- Retrospective analysis of CCTA in 158 patients undergoing percutaneous coronary intervention (PCI) and 62 controls.
- Assessed high-risk plaque characteristics (HRPCs) and hemodynamic parameters (FFRCT, ΔFFRCT, ischemic myocardial mass).
- Compared prognostic impact of clinical factors, HRPCs, and hemodynamics on subsequent coronary events.
Main Results:
- Vessels with events showed more HRPCs, lower FFRCT, higher ΔFFRCT, and higher ischemic myocardial mass.
- HRPCs and hemodynamic parameters demonstrated superior discriminant abilities compared to clinical risk factors.
- ΔFFRCT was the strongest predictor; HRPCs added incremental value to clinical factors, especially in vessels with negative FFRCT (>0.80).
Conclusions:
- CCTA-derived HRPCs and hemodynamic parameters offer superior prediction of subsequent coronary events than clinical risk factors alone.
- HRPCs provide additional predictive value to clinical risk factors in vessels with non-obstructive FFRCT.
