Summary

Digital evaluation of nuclear scores in thyroid lesions shows good agreement among pathologists, validating whole slide imaging for diagnosis. Proper validation prevents discordance between optical and digital methods.

Background:

  • Subjective nuclear feature evaluation in thyroid lesions causes diagnostic discrepancies.

Purpose of the Study:

  • To assess inter-pathologist concordance in nuclear score (NS) evaluation for thyroid lesions.
  • To compare optical versus digital assessment of NS using three scanning protocols.

Main Methods:

  • Three pathologists evaluated NS in 20 thyroid lesions using optical microscopy and three digital scanning protocols (P1000_20x, P1000_40x, DP200_20x).
  • Concordance was analyzed using Kappa statistic (κ) and intraclass correlation coefficient (ICC).

Main Results:

  • Good inter-pathologist agreement was observed in optical NS evaluation (ICC=0.73).
  • Digital assessment showed almost perfect agreement with optical for P1000_20x (κ=0.85) and substantial agreement for P1000_40x (κ=0.69) and DP200_20x (κ=0.77).
  • The P1000_20x protocol demonstrated the highest intra-pathologist concordance with optical evaluation.

Conclusions:

  • Digital whole slide imaging (WSI) is a valid method for NS evaluation in thyroid lesions.
  • Good agreement exists between pathologists and between optical and digital methods.
  • Validation procedures are crucial to minimize diagnostic discordance in digital pathology workflows.
