Peroneus longus intra-tendinous ganglion cyst

• A 38-year-old female presented with a year of right ankle swelling and pain.
• Magnetic Resonance Imaging (MRI) identified an intra-tendinous ganglion cyst within the peroneus longus tendon, located below the lateral malleolus.
• The cyst had a thin, well-defined wall.

Findings:

• Surgical excision of the intra-tendinous ganglion cyst was performed.
• The surgical technique successfully preserved the structural integrity of the peroneus longus tendon.
• Post-operative follow-up at one year showed satisfactory clinical and functional outcomes with no cyst recurrence.

Implications:

• This case highlights a rare presentation of intra-tendinous ganglion cysts.
• Successful surgical management can restore function and prevent recurrence in peroneus longus tendon cysts.
