Summary

Perianal tuberculosis (TB), a rare infection, can present as a persistent anal ulcer. Early diagnosis through biopsy and PCR, even in immunocompetent individuals, is crucial for effective anti-TB treatment.

Background:

  • Perianal tuberculosis (TB) is an exceptionally rare manifestation of mycobacterial infection.
  • It typically arises from autoinoculation secondary to active gastrointestinal TB.

Observation:

  • A case report details a 73-year-old immunocompetent male with a refractory perianal ulcer.
  • Initial TB tests and patient history were negative; however, chest X-ray revealed upper lung opacities.
  • Diagnosis was confirmed via biopsy and Polymerase Chain Reaction (PCR) after initial negative results.

Findings:

  • Histological examination and PCR confirmed perianal TB despite negative initial tests.
  • The patient's ulcer resolved completely with intensive anti-TB therapy.

Implications:

  • Perianal TB should be considered in refractory perianal ulcers, even in immunocompetent patients without a TB history.
  • Repeated biopsies and microbiological tests are vital for suspicious lesions.
  • Early antitubercular treatment is essential to prevent morbidity and manage this rare condition effectively.
