Primary hyperparathyroidism related hypercalcemic crisis (PHHC) is a serious condition. Early identification of PHHC in patients with high calcium, PTH, ALP, and creatinine levels is crucial for timely, life-saving treatment.

- To identify clinical features differentiating hypercalcemic crisis (HC) in primary hyperparathyroidism (PH) patients.
- To compare clinical and biochemical profiles of PH patients with and without HC.

- Retrospective study of 119 patients operated for PH between March 2015 and March 2020.
- Patients were divided into HC (serum calcium > 14 mg/dl) and non-HC groups.
- Comparison of demographic, biochemical, localization, histopathological, and postoperative data.

- HC group showed significantly higher serum Parathormone (PTH), calcium (Ca), Alkaline phosphatase (ALP), and creatinine levels.
- Hyper-functional parathyroid tumor size was significantly larger in the HC group.

Conclusions:
- PHHC requires more urgent treatment than elective PH surgery due to its potential mortality.
- Prioritize PH patients with elevated serum PTH, ALP, creatinine, and larger parathyroid tumors for prompt treatment to mitigate HC risk.
