    Background:

    • Mediastinal cysts are uncommon lesions originating from mediastinal structures.
    • These cysts can be congenital (e.g., bronchogenic, enteric, celomic) or acquired (e.g., thoracic duct cysts, lymphangiomas).
    • Their rarity and varied origins can lead to diagnostic difficulties.

    Purpose of the Study:

    • To evaluate the key characteristics of mediastinal cysts.
    • To analyze findings from a single institution's experience over an 11-year period.

    Main Methods:

    • A retrospective descriptive study was conducted from January 2009 to March 2020.
    • Included were 52 completely resected mediastinal cysts with available gross and microscopic features.
    • No post-operative complications were reported.

    Main Results:

    • Bronchogenic cysts were the most prevalent (57.69%), followed by thymic cysts (40.38%) and pericardial cysts (1.92%).
    • Microscopic examination confirmed the definitive diagnosis for all cases.
    • Radiologic findings correlated with the final diagnosis in only 28% of cases.

    Conclusions:

    • Microscopic analysis of the cystic wall is essential for accurate diagnosis of mediastinal cysts.
    • While characteristic locations exist (e.g., pericardial in cardiophrenic angle, thymic in thymic region, bronchogenic in middle mediastinum), cysts can appear anywhere.
    • Diagnostic challenges persist when characteristic features are absent, highlighting the importance of histopathology.
