Summary

Surgical reconstruction is recommended for neglected Monteggia fractures in children, with ulna osteotomies being effective. Gradual ulnar lengthening may be better for significant radius overgrowth.

  • Monteggia fractures are rare pediatric injuries often missed initially.
  • Delayed diagnosis can lead to chronic radial head dislocation and complications.
  • Surgical reconstruction is indicated for neglected Monteggia fractures.

Purpose of the Study:

  • To analyze the experience of a single tertiary center in treating neglected Monteggia fractures in children.
  • To evaluate the outcomes of surgical reconstruction for missed Monteggia fractures.

Main Methods:

  • Retrospective study of 13 pediatric patients with missed Monteggia fractures.
  • Analysis of hospital records, operative charts, follow-up data, and X-rays.
  • Radiographic results graded (good, moderate, poor); functional outcomes assessed using Mayo Elbow Performance Index.

Main Results:

  • 13 children (mean age 7.15 years) underwent surgery.
  • Treatments included ulna osteotomies (angulation or shortening) and ligament reconstruction or repair.
  • Good or excellent radiographic and functional outcomes were achieved in most patients (9/13).

Conclusions:

  • Surgical reconstruction of neglected pediatric Monteggia fractures is advisable.
  • Ulna osteotomies are suitable for most cases; Ilizarov method for significant radius overgrowth.
  • Annular ligament reconstruction is not always necessary.
