Summary

Transcatheter aortic valve replacement (TAVR) offers an alternative for severe aortic stenosis when the transfemoral approach is not possible. Transaortic TAVR provides a viable option, especially for patients with complex aortic anatomy, with procedural tips enhancing safety and speed.

Area of Science:

  • Cardiovascular Medicine
  • Interventional Cardiology
  • Surgical Innovation

Background:

  • Transfemoral transcatheter aortic valve replacement (TAVR) is the standard for severe aortic stenosis.
  • The transfemoral approach may not be feasible in all patients.
  • Alternative access routes are crucial for expanding TAVR accessibility.

Purpose of the Study:

  • To present transaortic TAVR as an alternative access strategy.
  • To highlight the utility of transaortic TAVR in patients with challenging thoracoabdominal aortas.
  • To share procedural tips for safe and efficient transaortic TAVR.

Main Methods:

  • Transaortic TAVR performed via J mini-sternotomy or right anterolateral mini-thoracotomy.
  • Focus on specific techniques and strategies for optimal access and valve deployment.

  • Case examples or procedural descriptions illustrating the approach.
  • Main Results:

    • Transaortic TAVR is a feasible alternative when transfemoral access is contraindicated.
    • The described techniques facilitate safe and efficient valve implantation.
    • Successful outcomes can be achieved in patients with complex aortic anatomy.

    Conclusions:

    • Transaortic TAVR is a valuable option for severe aortic stenosis patients unsuitable for transfemoral access.
    • Mini-sternotomy and mini-thoracotomy approaches offer effective alternatives.
    • Adherence to specific procedural tips can ensure a fast and safe transaortic TAVR procedure.
