Summary

Open reduction and internal fixation (ORIF) and radial head arthroplasty (RHA) yield similar long-term functional outcomes for operatively treated radial head fractures. Patient-reported results were comparable between the two surgical methods, including for complex fractures.

Purpose of the Study:

  • To compare long-term patient-reported functional outcomes after operative treatment of radial head fractures.
  • To evaluate differences between ORIF and RHA in functional recovery and secondary outcomes.

Main Methods:

  • A cross-sectional study identified patients with radial head fractures treated with ORIF or RHA between 2006 and 2018.
  • A survey in 2020 assessed long-term outcomes using the Quick Disabilities of the Arm, Shoulder, and Hand (QuickDASH) score.

Main Results:

  • Seventy-six patients completed the survey; QuickDASH scores were similar between ORIF (mean=15.7) and RHA (mean=22.8) groups (P=.97).
  • Rates of pain medication need and additional surgeries were not significantly different between the ORIF and RHA groups.
  • Subgroup analysis of multi-fragmentary fractures showed similar outcomes for both treatment groups.

Conclusions:

  • Patient-reported functional outcomes were comparable between ORIF and RHA at an average of 7.5 years post-surgery.
  • Radial head reconstruction may not result in worse outcomes than RHA when chosen appropriately by the surgeon.
