Intranodal embolization effectively treats persistent groin lymphoceles. This minimally invasive procedure using n-butyl cyanoacrylate glue demonstrated a 100% success rate with no complications in a recent study.

Area of Science:

  • Vascular Surgery
  • Interventional Radiology
  • Lymphatic Medicine

Background:

  • Persistent lymphatic leakage, particularly groin lymphocele, poses a significant clinical challenge.
  • Current treatment options for persistent lymphatic leakage may be invasive or have limited efficacy.
  • Intranodal embolization is an emerging minimally invasive technique for lymphatic leakage.

Purpose of the Study:

  • To describe the procedural details of intranodal embolization for groin lymphocele.
  • To evaluate the efficacy and safety of intranodal embolization for persistent groin lymphocele.
  • To assess the technical and clinical success rates of intranodal embolization.

Main Methods:

  • Retrospective chart review of nine consecutive patients with groin lymphocele treated between January 2017 and December 2019.

  • Intranodal lymphangiography with iodinated contrast to identify lymphatic leakage.
  • Embolization using n-butyl cyanoacrylate glue mixed with iodized oil.
  • Main Results:

    • The primary etiologies of lymphocele were iatrogenic (inguinal vessel access, lymph node biopsy) and trauma.
    • 100% technical and clinical success rates were achieved for both lymphangiography and intranodal embolization.
    • The median treatment duration was 2 days, with no acute or chronic complications observed during a median follow-up of 9 days.

    Conclusions:

    • Intranodal embolization is a promising treatment for persistent groin lymphocele, offering a high success rate and short treatment duration.
    • The procedure using n-butyl cyanoacrylate glue is safe and effective, with no observed complications.
    • Further research is warranted to validate these findings and establish intranodal embolization as a standard treatment option.
