    Background:

    • A 51-year-old female patient on amiodarone developed a painful thyroid enlargement after ten months of treatment.
    • Elevated thyroid antibody titers and lymphocytic/plasmacytic infiltration were observed in the thyroid fine needle aspirate.

    Findings:

    • The patient initially presented with hyperthyroidism.
    • Subsequently, she developed hypothyroidism necessitating thyroid hormone substitution therapy.

    Implications:

    • This case suggests amiodarone may provoke autoimmune thyroiditis.
    • Clinicians should consider amiodarone-induced autoimmune thyroiditis in patients presenting with thyroid abnormalities.
    • Monitoring thyroid function and antibody levels is crucial for patients on long-term amiodarone therapy.
