Background:

  • Secondary rhinoplasty for unilateral cleft lip and palate (UCLP) requires accurate preoperative diagnosis of nasal deformities.
  • Currently, no standardized outcome measurement tool exists for assessing UCLP nose sequelae prior to secondary rhinoplasty.

Purpose of the Study:

  • To identify and rate nasal deformities in UCLP patients.
  • To develop and validate an evaluation scale for collecting and analyzing cleft nose data.

Main Methods:

  • Retrospective cohort study of 29 UCLP patients undergoing secondary rhinoplasty (2010-2021).
  • Preoperative 2D photography assessed using a custom 16-item scale with a binary scoring system by two experts.
  • Inter-examiner reliability assessed using the intraclass correlation coefficient (ICC).

Main Results:

  • The most frequent deformities were alar foot displacement (93%), enlarged nasal tip (90%), and nostril horizontalization (86%).
  • The custom assessment scale demonstrated strong inter-examiner reliability (ICC = 0.911, P<0.05).

  • The proposed photographic assessment system is simple, reliable, and reproducible for diagnosing nasal deformities in UCLP patients.
  • This tool can aid clinicians in preoperative diagnosis, postoperative success evaluation, and comparison of surgical techniques for secondary rhinoplasty.
