Background:

Odontogenic maxillary sinusitis often recurs after endoscopic sinus surgery due to poor drainage.
Traditional Caldwell-Luc surgery for sinusitis can lead to maxillary sinus deformity and dysfunction.
Current management strategies for recurrent sinusitis are inadequate.

Observation:

A case of severe odontogenic maxillary sinusitis with frontal sinus involvement was treated.
A conservative surgical technique involved preparing a nasoantral window via the Caldwell-Luc approach.
Maxillary sinus mucosa was preserved during the procedure.

Findings:

The novel conservative management facilitated complete recovery in a severe sinusitis case.
Preserving sinus mucosa and ensuring adequate drainage improved patient outcomes.
This approach potentially prevents recurrence and complications associated with radical surgery.

Implications:

This conservative management may offer a superior alternative for treating odontogenic maxillary sinusitis.
Further studies are warranted to validate this technique in a larger patient cohort.
This approach could minimize long-term morbidity and improve quality of life for sinusitis patients.
