Medical trainees perceive burnout as a spectrum of negative experiences, while wellbeing is a dynamic reservoir. Value fulfillment is key to preventing burnout by maintaining this reservoir.
Area of Science:
- Medical Education
- Psychology
- Occupational Health
Background:
- Limited research exists on medical trainees' personal experiences of burnout.
- Theoretical understanding of trainee wellbeing and its link to burnout is underdeveloped.
Purpose of the Study:
- To conceptualize burnout and wellbeing among medical trainees.
- To explore the relationship between trainee wellbeing and burnout.
Main Methods:
- Grounded theory approach with a post-positivist epistemology.
- Concurrent data collection and analysis using constant comparison and triangulation.
- Semi-structured interviews with trainees and focus groups with supervisors/educators.
Main Results:
- Burnout described as a spectrum with seven themes: altered emotion, compromised performance, disengagement, dissatisfaction, exhaustion, overexertion, and feeling overwhelmed.
- Wellbeing comprises interacting personal and professional domains, fueled by a 'reservoir'.
- Burnout occurs when the wellbeing reservoir is depleted, linked to value fulfillment.
Conclusions:
- Burnout and wellbeing are multifaceted, interconnected constructs in medical trainees.
- Interventions should address both individual and workplace factors, focusing on value fulfillment.
- A novel model is proposed, synthesizing and advancing existing research on trainee burnout and wellbeing.
