    Summary

Chronic lymphocytic leukemia (CLL) is rare in China. A Peking Union Medical College study found 4.6% of leukemia patients had CLL, suggesting a genetic basis for its low incidence in Chinese populations.

    Area of Science:

    • Hematology
    • Oncology
    • Epidemiology

    Background:

    • Chronic lymphocytic leukemia (CLL) is prevalent in Western countries but notably rare in Asian populations, including Chinese individuals.
    • Existing data on CLL frequency in China is primarily from the Chinese literature, necessitating further review of available records.

    Purpose of the Study:

    • To investigate the frequency of CLL among adult leukemia patients at Peking Union Medical College (PUMC).
    • To compare clinical and laboratory aspects of CLL in Chinese patients with those from Western populations, specifically the University of Utah.

    Main Methods:

    • Retrospective review of 4,174 adult leukemia patient records at PUMC from 1952-1986.
    • Analysis of CLL frequency in comparison to other leukemia types (CML, AML, ALL).
    • Comparative analysis of clinical and laboratory data between PUMC and University of Utah CLL patient cohorts.

    Main Results:

    • CLL accounted for 4.6% of leukemia cases at PUMC, with other types being more frequent: CML (22.9%), AML (50.3%), and ALL (22.3%).
    • The frequency of CLL in the PUMC cohort was higher than in other Chinese multi-institutional collections but significantly lower than in Western countries.
    • No substantial differences in the overall nature of CLL were observed between Chinese and USA patients, though symptom duration before diagnosis was longer in Chinese patients.

    Conclusions:

    • The low incidence of CLL in China is not explained by differences in lifespan or the prevalence of other leukemia types.
    • A genetic basis is proposed as the most likely explanation for the rarity of CLL in the Chinese population, similar to hypotheses for Japanese populations.
