Extraskeletal osteosarcoma (ESOS) is a rare cancer presenting outside bone. This report details an unusual case of ESOS within a bladder diverticulum in an elderly male patient.

Background:

Extraskeletal osteosarcoma (ESOS) is a rare malignant tumor constituting only 1% of all sarcomas.
ESOS originates in soft tissues, distinct from primary bone osteosarcoma.
Primary osteosarcoma within the bladder is exceptionally rare.

Observation:

A 78-year-old male presented with a palpable right lower quadrant mass.
Imaging and subsequent biopsy revealed extraskeletal osteosarcoma (ESOS).
The ESOS was found originating within a bladder diverticulum.

Findings:

This case represents extraskeletal osteosarcoma (ESOS) of the bladder.
Fewer than 50 cases of ESOS in the bladder have been documented globally.
This is the fourth reported instance of primary osteosarcoma arising within a bladder diverticulum.

Implications:

Highlights the importance of considering rare diagnoses in soft tissue masses.
Contributes to the limited literature on bladder-specific extraskeletal osteosarcoma.
Underscores the need for thorough histopathological evaluation of bladder diverticulum masses.
