Summary

Exudative retinal detachment (ERD) is a rare complication of retinopathy of prematurity (ROP) laser treatment. Tailoring laser settings and adjusting postoperative care can lead to near-complete resolution of ERD.

Area of Science:

  • Ophthalmology
  • Neonatology

Background:

  • Retinopathy of prematurity (ROP) is a leading cause of blindness in premature infants.
  • Laser photocoagulation is a standard treatment for ROP, but can have complications.
  • Exudative retinal detachment (ERD) is a rare but serious postoperative complication of ROP laser treatment.

Observation:

  • A case of bilateral ERD occurred in a preterm infant following ROP laser photocoagulation.
  • The infant received standard laser treatment with adjusted power due to absorption.
  • Postoperative care included prednisolone acetate drops, which were later intensified.

Findings:

  • Complete ERD developed one week after laser treatment.
  • ERD resolved near-completely within eight weeks with modified postoperative management.

  • The case highlights the importance of individualized laser settings and titration.
  • Implications:

    • This case underscores the need for personalized laser parameters in ROP treatment, even with established protocols.
    • Close monitoring and titration of laser power based on tissue response are crucial.
    • Effective management of postoperative ERD is achievable with adjustments to standard care.
