Background:

  • Palliative care (PC) is recommended for amyotrophic lateral sclerosis (ALS) patients, yet evidence on optimal delivery is limited.
  • This study describes a pilot program integrating longitudinal, interdisciplinary PC into ALS patient care.

Purpose of the Study:

  • To evaluate the structure and impact of an interdisciplinary palliative care program for patients with amyotrophic lateral sclerosis (ALS).
  • To assess the feasibility and effectiveness of PC in addressing diverse patient and family needs.

Main Methods:

  • An observational cohort study of 55 patients with ALS referred to outpatient PC between October 2017 and July 2020.
  • Patients received a mean of 5 scheduled PC visits, often via video, involving a multidisciplinary team.

Main Results:

  • PC addressed non-pain symptoms (94.5%), psychosocial distress (78.2%), and care planning (96.4%), significantly increasing advance directives and POLST completion rates.
  • The program supported family caregivers (96.4%), with 77.8% of deceased patients utilizing hospice.
  • Aid-in-dying was obtained by 11 patients, with 8 utilizing it.

Conclusions:

  • Integrating longitudinal, interdisciplinary PC into ALS care is feasible and beneficial, addressing multiple domains and enhancing advance care planning.
  • Further controlled studies are necessary to fully understand the impact of PC on ALS patients, families, and clinicians.
