Summary

Nearly 20% of older Cerebral Autosomal Dominant Arteriopathy with Subcortical Infarcts and Leukoencephalopathy (CADASIL) patients remain neurologically intact. These individuals show distinct clinical and imaging features compared to symptomatic patients, suggesting unique preservation mechanisms.
