    Purpose of the Study:

    • To evaluate the impact of standardized criterion-based (CB) interpretation of renal ultrasounds (RUS) in infants aged ≤60 days with UTIs.
    • To compare the diagnostic yield of CB interpretation versus non-criterion-based (NCB) interpretation for renal imaging abnormalities.
    • To assess the utility of follow-up imaging based on standardized RUS findings.

    Main Methods:

    • Retrospective analysis of infants ≤60 days hospitalized with UTI or fever and available urine culture and renal imaging.
    • Reinterpretation of initial non-criterion-based (NCB) RUS by a single radiologist using a criterion-based (CB) hydronephrosis grading system.
    • Comparison of renal imaging outcomes between NCB and CB interpretation groups.

    Main Results:

    • Of 193 infants, 114 had abnormal NCB RUS interpretations.
    • CB reinterpretation classified 75% of these as minor and 25% as significant abnormalities.
    • Significant CB abnormalities correlated with higher rates of abnormal follow-up imaging and high-grade reflux on voiding cystourethrogram (VCUG).

    Conclusions:

    • Standardized criterion-based (CB) RUS interpretation improves the accuracy of identifying renal abnormalities in young infants with UTIs.
    • Follow-up RUS has limited utility for infants with minor CB abnormalities, but follow-up VCUG remains valuable for detecting high-grade reflux.
