Robot-assisted pyeloplasty (RAP) and laparoscopic pyeloplasty (LP) offer comparable success rates for uretero-pelvic junction obstruction (UPJO) in adults. While RAP involves longer operative times, it results in shorter hospital stays and similar complication rates to LP.

Area of Science:

  • Urology
  • Minimally Invasive Surgery

Background:

  • Uretero-pelvic junction obstruction (UPJO) is commonly treated with minimally invasive techniques.
  • Laparoscopic pyeloplasty (LP) and robot-assisted pyeloplasty (RAP) are established surgical options.

Purpose of the Study:

  • To compare the clinical outcomes of RAP versus LP for adult UPJO.
  • To evaluate differences in operative time, hospital stay, complications, and success rates.

Main Methods:

  • Retrospective analysis of adult patients who underwent RAP (n=41) or LP (n=24) for UPJO.
  • Comparison of demographic data, operative parameters, and postoperative outcomes.

Main Results:

  • Demographics were comparable between groups.
  • RAP had significantly longer operative times (total and skin-to-skin) but shorter hospital stays.

  • Postoperative complication rates were similar, and success rates were equivalent (90.6% for RAP vs. 87.5% for LP).
  • Conclusions:

    • Robot-assisted pyeloplasty provides equivalent clinical outcomes to laparoscopic pyeloplasty in adult UPJO.
    • Longer operative times with RAP may be associated with its ability to manage more complex cases.
