    Summary

    Geographic helicoid peripapillary choroidopathy (GHPC) can manifest in the macula. Prompt corticosteroid treatment of this macular form of GHPC led to remission and vision preservation in all patients.

    Background:

    • Geographic helicoid peripapillary choroidopathy (GHPC), also known as serpiginous choroidopathy, typically affects the peripapillary retina.
    • Atypical presentations of GHPC initially or exclusively involving the macula are less commonly described.

    Observation:

    • Eight patients presented with macular lesions clinically and angiographically similar to GHPC.
    • These macular lesions exhibited recurrence and, in some cases, progression to the peripapillary region or involvement of the fellow eye.

    Findings:

    • The macular presentation is suggested to be a variant manifestation of GHPC.
    • Corticosteroid therapy administered promptly for acute episodes (initial or recurrent) resulted in disease remission.
    • Vision was preserved in all eight treated eyes.

    Implications:

    • This study expands the understanding of GHPC's clinical spectrum to include macular involvement.
    • Early diagnosis and treatment with corticosteroids may be crucial for managing this macular variant of GHPC.
    • Further research is needed to identify the causative agent of GHPC.
