Lipoma causing ileocecal intussusception and its endoscopic resection

Background:

• Intussusception is uncommon in adults and can present as acute mechanical intestinal obstruction (AMIO).
• AMIO is often not considered in the differential diagnosis for adult intussusception.
• Ileocecal intussusception, a rare cause of AMIO, was managed endoscopically in this case.

Observation:

• A 58-year-old male presented with symptoms of AMIO, including nausea, vomiting, abdominal pain, and distention.
• Radiological assessments identified ileocecal intussusception caused by an ileal lipoma.
• The intussuscepted segment was reduced endoscopically, and the lipoma was resected colonoscopically.

Findings:

• Successful endoscopic pneumatic reduction (ER) of the intussuscepted segment.
• Complete endoscopic resection of the ileal lipoma without surgical intervention.
• The patient experienced no complications and was discharged following the procedure.

Implications:

• Endoscopic resection is a safe and effective treatment for intussusception when malignancy is excluded.
• This minimally invasive approach avoids unnecessary surgery, reducing patient discomfort and healthcare costs.
• Endoscopic management offers a viable alternative to surgery for select cases of adult intussusception.
