  • A 35-year-old male presented with bilateral blurred vision and multiple yellow-white punctate lesions in the posterior pole.
  • Ocular multimodal imaging revealed cystoid macular edema, submacular edema, ellipsoid zone thickening, and enhanced macular reflex.
  • Fundus autofluorescence showed hyperautofluorescence, while fluorescein angiography demonstrated no leakage.

Findings:

  • The patient was diagnosed with AEPVM based on clinical presentation and multimodal imaging.
  • Despite no specific treatment, the patient experienced lesion fusion, expansion, and worsening macular edema within one week.

Implications:

  • This case underscores the potential for rapid progression in AEPVM, even without immediate intervention.
