Summary

Transoral endoscopic thyroidectomy vestibular approach (TOETVA) offers a safe and effective minimally invasive option for thyroid nodules and papillary thyroid carcinoma. This technique provides excellent aesthetic outcomes with minimal complications.

Background:

Minimally invasive thyroid surgery is widely accepted.
The transoral endoscopic thyroidectomy vestibular approach (TOETVA) is a novel technique.
Aesthetic outcomes are increasingly important in thyroid surgery.

Purpose of the Study:

To evaluate the safety and efficacy of TOETVA.
To assess TOETVA with or without central neck dissection.
To analyze outcomes in patients with benign nodules and papillary thyroid carcinoma (PTC).

Main Methods:

A case series of 103 patients undergoing TOETVA between December 2018 and December 2021.
76 patients had benign nodules, 27 had PTC.
Central neck dissection was performed in patients with PTC.

Main Results:

No conversions to open surgery were required.
Average operative time was 116.5 minutes; median blood loss was 40.1 mL.
Temporary hoarseness (8.7%) and temporary chin numbness were the main complications.

Conclusions:

TOETVA is a safe and effective surgical option.
The technique provides highly aesthetic results.
TOETVA is suitable for both benign thyroid nodules and PTC, with or without central neck dissection.
