Summary

Transoral endoscopic thyroidectomy vestibular approach (TOETVA) offers a safe and effective minimally invasive option for thyroid nodules and papillary thyroid carcinoma. This technique provides excellent aesthetic outcomes with minimal complications.

Background:

  • Minimally invasive thyroid surgery is widely accepted.
  • The transoral endoscopic thyroidectomy vestibular approach (TOETVA) is a novel technique.
  • Aesthetic outcomes are increasingly important in thyroid surgery.

Purpose of the Study:

  • To evaluate the safety and efficacy of TOETVA.
  • To assess TOETVA with or without central neck dissection.
  • To analyze outcomes in patients with benign nodules and papillary thyroid carcinoma (PTC).

Main Methods:

  • A case series of 103 patients undergoing TOETVA between December 2018 and December 2021.
  • 76 patients had benign nodules, 27 had PTC.
  • Central neck dissection was performed in patients with PTC.

Main Results:

  • No conversions to open surgery were required.
  • Average operative time was 116.5 minutes; median blood loss was 40.1 mL.
  • Temporary hoarseness (8.7%) and temporary chin numbness were the main complications.

Conclusions:

  • TOETVA is a safe and effective surgical option.
  • The technique provides highly aesthetic results.
  • TOETVA is suitable for both benign thyroid nodules and PTC, with or without central neck dissection.
