Background:

  • Penile plication (PP) is a surgical treatment for Peyronie's Disease (PD).

Purpose of the Study:

  • To reassess penile length changes following PP using a novel measurement of functional penile length (FPL).
  • To compare preoperative and postoperative FPL measurements in PD patients undergoing PP.

Main Methods:

  • A prospective database of PD patients who underwent PP between 2018-2021 was queried.
  • Functional penile length (FPL) was defined and measured intraoperatively on erect penises from the pubic base to the glans tip, disregarding curvature.
  • Measurements were taken before incision and immediately after surgical correction.

Main Results:

  • Twenty-eight PD patients were included (average age 59, SHIM 17, PDQ 9).
  • Mean preoperative FPL was 11.1 ± 1.4 cm, increasing to 12.5 ± 1.3 cm postoperatively, an average increase of 12.8% ± 6.6%.
  • All patients reported subjective improvement in penile curvature.

Conclusions:

  • Functional penile length (FPL) is a valuable outcome measure for PP in PD patients.
  • PP can result in an increase in FPL, challenging the notion of it being solely a shortening procedure.
  • This finding aids in preoperative counseling and setting realistic patient expectations regarding penile length post-surgery.
