Summary

Primary biliary cirrhosis (PBC) can affect the lungs, causing symptoms like fever, cough, and dyspnea. These cases show lung tissue may be another autoimmune target in PBC patients.

Background:

  • Primary biliary cirrhosis (PBC) is an autoimmune liver disease associated with Sjogren's syndrome.
  • PBC involves a complex autoimmune disorder of the secretory immune system and epithelial end organs.
  • Pulmonary manifestations in PBC are not well-characterized.

Observation:

  • Four patients with PBC and Sjogren's syndrome presented with significant pulmonary disease.
  • Symptoms included fever, cough, and dyspnea.
  • Diverse lung pathologies were observed: obstructive lung disease, interstitial lung disease, pulmonary nodules, and granulomas.

Findings:

  • One patient exhibited obstructive lung disease linked to an autoimmune process, confirmed by lung parenchyma immunofluorescence.
