Background:

  • Prostate cancer (PCa) diagnosis in patients with gray-zone prostate-specific antigen (PSA) levels (4-10.0 ng/mL) presents a clinical challenge.
  • Accurate prediction is crucial to avoid unnecessary prostate biopsies.

Purpose of the Study:

  • To develop a predictive nomogram for PCa in patients with gray-zone PSA levels.
  • To integrate radiomics and clinical parameters for improved diagnostic accuracy.

Main Methods:

  • Retrospective study of 274 patients with gray-zone PSA levels, divided into training (n=191) and validation (n=83) sets.
  • Development of radiomics, clinical, and combined prediction models using logistic regression.
  • Nomogram construction incorporating radiomics signature, Prostate Imaging Reporting and Data System (PI-RADS) V2.1 category, age, and prostate volume.

Main Results:

  • The combined nomogram achieved an AUC of 0.953, outperforming the radiomics model (AUC: 0.941) and clinical model (AUC: 0.866).

  • This approach can help reduce unnecessary prostate biopsies in patients with gray-zone PSA levels.
