Background:

Purpose of the Study:

  • To investigate the link between ischemic stroke topography on diffusion-weighted MRI and stroke etiology in cancer patients.
  • To analyze stroke patterns in relation to cancer diagnoses.

Main Methods:

  • Retrospective analysis of ischemic stroke patients from the Bezmialem Stroke Registry (4-year period).
  • Inclusion criteria: acute ischemic stroke with active solid malignancy (excluding hematologic).
  • Correlation of stroke etiology with diffusion restriction patterns on MRI.

Main Results:

  • 32 of 1472 patients had active cancer; stroke etiology was primarily embolic/cardioembolic (22/32).
  • 15 patients had isolated acute infarction; 17 had multiple infarct areas.
  • Multiple infarcts often showed bilateral microembolisms in over one vessel area (13/17).

Conclusions:

  • Embolic/cardioembolic events are the leading cause of ischemic stroke in cancer patients.
  • This finding significantly impacts treatment strategies for stroke in this population.
