Summary
This study reports the first simultaneous robot-assisted radical nephroureterectomy and cystectomy in Japan for a patient with muscle-invasive bladder cancer and upper tract urothelial carcinoma. The procedure was successfully completed with no complications.

- Simultaneous RANU and RARC is a feasible surgical option for select patients with concurrent MIBC and UTUC.
- This approach may offer benefits in terms of surgical efficiency and resource utilization.
- Further research is warranted to evaluate long-term outcomes and oncological control.
