Background:

Pregnancy outcomes in women with COVID-19 are varied.
Severe intrauterine growth restriction (IUGR) and abnormal fetal Doppler flows can indicate placental compromise.
The impact of second-trimester SARS-CoV-7 (Severe Acute Respiratory Syndrome Coronavirus 2) infection on placental function requires further investigation.

Observation:

A pregnant woman in her second trimester contracted COVID-19, presenting with absent fetal movements.
The fetus exhibited severe intrauterine growth restriction and abnormal fetal Doppler flows.
Despite initial concerns, placental function showed gradual improvement over several months.

Findings:

The pregnancy was successfully continued until 35 weeks gestation, culminating in an acute cesarean section.
The placenta tested negative for SARS-CoV-2 nucleocapsid, suggesting no direct viral invasion.
The neonate tested positive for COVID-19 at four days old but remained asymptomatic.

Implications:

This case highlights the potential for spontaneous improvement in placental function despite severe COVID-19 infection during pregnancy.
It underscores the importance of close monitoring in pregnancies complicated by maternal infections.
Further research is needed to understand the mechanisms behind fetal recovery and asymptomatic neonatal COVID-19 infection.
