Rectus femoris tendon ruptures in young athletes can be debilitating. Surgical repair with Achilles allograft offers a viable solution for those unresponsive to conservative care, restoring function and reducing pain.

Background:
- Quadriceps ruptures, particularly isolated rectus femoris tendon ruptures, are rare in young, healthy individuals.
- Persistent pain and dysfunction can occur despite non-operative management for these injuries.
Observation:
- An 18-year-old collegiate sprinter presented with a rectus femoris tendon rupture, femoroacetabular impingement, and a labrum tear.
- Non-operative management failed to alleviate the patient's significant pain and functional limitations.
Findings:
- The patient underwent surgical repair of the rectus femoris rupture using an Achilles allograft.
- Arthroscopic labrum fixation and femoroplasty were performed concurrently.
- Post-surgery, the patient returned to sport with improved mobility and reduced pain.
Implications:
- Surgical reconstruction of rectus femoris tendon ruptures with Achilles allograft is an effective treatment option.
- This approach can successfully treat combined rectus femoris ruptures with femoroacetabular impingement and labrum tears.
- Operative intervention can lead to successful return to sport for young athletes with complex quadriceps injuries.
