Background:

Appendiceal diverticulitis is an uncommon condition with an unclear etiology.
Inflammatory Bowel Disease (IBD) is a chronic inflammatory condition affecting the gastrointestinal tract.

Observation:

A 41-year-old female patient with a history of IBD presented with right iliac fossa pain.
Diagnostic workup revealed acute appendiceal diverticulitis upon histopathologic examination.

Findings:

This case presents the first documented instance of appendiceal diverticulitis in a patient with a history of IBD.
The findings suggest a potential, previously undescribed, causative relationship between IBD and appendiceal diverticulitis.

Implications:

This study highlights the need for further research into the etiology of appendiceal diverticulitis.
Considering IBD as a potential contributing factor may influence diagnostic and treatment strategies for appendiceal diverticulitis.
