  • Acute aortic dissections (AAD) carry significant morbidity and mortality.
  • American Heart Association (AHA) guidelines recommend strict systolic blood pressure (SBP) and heart rate (HR) control for type B AAD.
  • Predictors for successful emergency department (ED) SBP management in type B AAD are not well-established.

Purpose of the Study:

  • To identify predictors of achieving SBP and HR goals in patients with type B AAD in the ED.
  • To evaluate adherence to AHA guidelines for hemodynamic management prior to transfer.

Main Methods:

  • Retrospective analysis of 134 adult patients with type B AAD transferred between 2017-2020.
  • Inclusion criteria: initial SBP >120 mmHg and HR >60 bpm.
  • Random forest (RF) algorithms used to predict achievement of SBP <120 mmHg and HR <60/80 bpm.

Main Results:

  • Only 19% of patients achieved SBP <120 mmHg at ED departure.
  • Higher triage SBP and intravenous labetalol predicted SBP >120 mmHg, contradicting AHA guidelines.
  • Adequate pain management and timely CT scans predicted achievement of HR goals.

Conclusions:

  • Many type B AAD patients do not meet AHA hemodynamic goals in the ED before transfer.
  • Higher initial SBP and HR predict failure to achieve therapeutic goals.
  • Effective pain management may contribute to meeting HR targets.
