Black patients with Peripheral Artery Disease (PAD) face a higher risk of major amputation within 30 days post-intervention compared to White patients. However, overall mortality rates remain similar between racial groups following PAD procedures.

Purpose of the Study:
- To conduct a meta-analysis evaluating post-procedure outcomes in PAD patients, comparing Black and White racial groups.
- To identify significant differences in major amputation, myocardial infarction, and mortality rates between these groups.
Main Methods:
- Systematic search of electronic databases until November 2022.
- Meta-analysis of 136,395 PAD patients (19,218 Black, 117,177 White).
- Primary endpoint: major amputation within 30 days. Secondary endpoints: myocardial infarction (MI), 30-day mortality, and all-cause mortality (ACM).
Main Results:
- Black patients had a significantly higher odds of major amputation within 30 days (OR, 0.40; P=0.02).
- White patients showed higher odds of MI within 30 days (OR, 1.29; P<0.02).
- No significant differences were found in ACM (OR, 0.97; P=0.88) or 30-day mortality (OR, 1.09; P=0.64).
Conclusions:
- Black PAD patients face a higher risk of major amputation within 30 days post-intervention compared to White patients.
- Mortality rates were comparable between Black and White PAD patients.
- This study represents the largest meta-analysis to date on racial disparities in PAD intervention outcomes.
