Summary

A large retroperitoneal mass in an elderly male was diagnosed as diffuse large B-cell lymphoma. Chemotherapy with R-THP-COP led to a partial response, with no signs of recurrence upon discharge.

Background:

  • Retroperitoneal masses can present with non-specific symptoms like anorexia.
  • Accurate diagnosis of retroperitoneal tumors requires a multidisciplinary approach involving imaging and histopathology.

Observation:

  • A 20 cm homogeneous retroperitoneal mass was identified via abdominal computed tomography (CT).
  • Elevated soluble interleukin-2 receptor and lactate dehydrogenase levels suggested malignancy.
  • Ultrasonography-guided biopsy revealed large lymphocytes positive for CD20 and CD79a.

Findings:

  • Histopathological analysis confirmed diffuse large B-cell lymphoma.
  • The patient underwent R-THP-COP chemotherapy (rituximab, pirarubicin, cyclophosphamide, vincristine, prednisolone).
  • Partial response was achieved after four chemotherapy cycles, per Response Evaluation Criteria in Solid Tumors (RECIST) guidelines.

Implications:

  • This case highlights the importance of prompt diagnosis and treatment of large retroperitoneal lymphomas.
  • R-THP-COP therapy demonstrated efficacy in managing this aggressive lymphoma subtype.
  • Successful treatment resulted in patient discharge with no evidence of disease recurrence.
