Recurrent fluid in age-related macular degeneration (AMD) most often appears centrally, even if initially elsewhere. Some patients experience new fluid locations, suggesting potential new disease activity.

Background:
- Neovascular age-related macular degeneration (nAMD) is a leading cause of vision loss.
- Subretinal fluid (SRF) and intraretinal fluid (IRF) are key indicators of disease activity in nAMD.
- As-needed (PRN) ranibizumab is a common treatment for nAMD, but fluid recurrence patterns require further investigation.
Purpose of the Study:
- To analyze the recurrence patterns of subretinal fluid and/or intraretinal fluid in patients with nAMD treated with PRN ranibizumab.
- To compare the location of fluid at baseline with the location of recurrence after a period of resolution.
Main Methods:
- A post hoc analysis of the HARBOR trial (NCT00891735) was performed.
- Included patients (N=222) with SRF and/or IRF at baseline and recurrence after ≥3 months of absence.
- Compared baseline fluid locations with recurrence locations.

- Recurrent fluid was most frequently located centrally (69%), unlike baseline fluid distribution.
- Eyes with baseline central fluid typically recurred centrally (72%).

- Recurrent fluid in nAMD predominantly occurs centrally, regardless of initial location.
