Summary

His bundle pacing (HBP) is safer with the KODEX-EPD nonfluoroscopic mapping system, significantly reducing fluoroscopic time (FT). This new technology lowers radiation exposure without increasing procedural time (PT).

Background:

  • His bundle pacing (HBP) can be technically demanding, often requiring prolonged fluoroscopic time (FT) and procedural time (PT).
  • Conventional HBP relies heavily on fluoroscopy for catheter navigation and lead placement.
  • Minimizing radiation exposure is a key goal in cardiac procedures.

Purpose of the Study:

  • To evaluate the efficacy of a new nonfluoroscopic mapping (NFM) system, the KODEX-EPD, in reducing FT and PT during HBP.
  • To compare outcomes between HBP guided by the KODEX-EPD system versus conventional fluoroscopy-guided methods.
  • To assess the safety and effectiveness of NFM in HBP procedures.

Main Methods:

  • A prospective study included 46 patients undergoing HBP.
  • Twenty-two patients (group 1) received NFM-guided HBP using the KODEX-EPD system.
  • Twenty-four patients (group 2) underwent conventional fluoroscopy-guided HBP. Lead types and pacing indications were recorded.

Main Results:

  • HBP success rates were high in both groups (100% in group 1, 96% in group 2).
  • Fluoroscopic time (FT) was significantly reduced in the NFM group (183 ± 117s) compared to the conventional group (464.1 ± 352s; p=0.012).
  • No significant differences were observed in procedural time (PT) or other procedural outcomes between the groups.

Conclusions:

  • The KODEX-EPD system can be safely and effectively used for His bundle pacing.
  • This NFM system significantly reduces ionizing radiation exposure by decreasing FT.
  • The use of the KODEX-EPD system does not prolong procedural time.
