    Background:

    • Connective tissue grafts (CTGs) are widely used in periodontal and oral surgery.
    • Graft deepithelialization techniques vary, with blade excision being conventional.
    • Predictable and complete epithelial removal is crucial for successful graft integration.

    Purpose of the Study:

    • To histologically compare two connective tissue graft harvesting techniques.
    • To evaluate the effectiveness of intraoral bur deepithelialization versus extraoral blade deepithelialization.

    Main Methods:

    • Three patients provided six standardized CTGs from the lateral palatal mucosa.
    • One technique involved intraoral deepithelialization with a round bur before harvesting.
    • The other technique involved harvesting first, followed by extraoral deepithelialization with a blade.

    Main Results:

    • No significant differences in graft thickness, proportion, or composition were observed between techniques.
    • Epithelial remnants were present in five of six grafts after deepithelialization.
    • Intraoral bur deepithelialization was more predictable, while blade excision offered better graft handling and regularity.

    Conclusions:

    • Complete epithelial removal by blade is challenging and often incomplete.
    • Blade excision provides advantages in graft handling and regularity.
    • Intraoral deepithelialization with a bur appears to be a more predictable method for epithelial removal in CTG harvesting.
