Background:

  • Progressive macular hypomelanosis (PMH) is a rare condition characterized by depigmented macules and patches.
  • Adolescent females are the primary demographic affected by PMH.
  • The exact etiology of PMH remains unclear, though a bacterial cause is suspected.

Observation:

  • Two adolescent females presented with asymptomatic, non-scaly, hypopigmented macules and patches on their arms.
  • Wood's lamp examination revealed punctiform coral-red perifollicular fluorescence on the back and faint intralesional enhancement.
  • Skin biopsy in one case confirmed the diagnosis of progressive macular hypomelanosis.

Findings:

  • The observed perifollicular fluorescence suggests a potential role for Cutibacterium acnes.
  • Antimicrobial therapy, combined with sun exposure, resulted in complete resolution of the hypopigmented lesions in one patient.
  • This indicates a potential therapeutic response to targeting bacterial involvement.

  • The findings support the hypothesis that Cutibacterium acnes may play a role in the pathogenesis of PMH.
  • Antimicrobial treatment could be a viable therapeutic option for progressive macular hypomelanosis.
