  • The patient later experienced acute decompensation 12 hours post-ingestion, characterized by autonomic instability and acute pulmonary edema.

Findings:

  • The secondary decompensation suggests a complex pathophysiological response beyond typical organophosphate poisoning.
  • This atypical clinical course highlights the challenges in managing exposures to adulterated pesticides.

Implications:

  • Understanding the delayed and dual toxidrome is crucial for effective clinical management of street pesticide ingestions.
  • Further research into the synergistic or antagonistic effects of mixed pesticide components is warranted.
