Background:

  • Postoperative confusion is a common complication after open-heart surgery.
  • The relationship between sleep disturbances and confusion in these patients is not fully understood.

Purpose of the Study:

  • To investigate the temporal relationship between sleep loss and the occurrence of confusion in patients undergoing open-heart surgery.

Main Methods:

  • 27 patients undergoing open-heart surgery were monitored.
  • Assessments included the Folstein Mini-Mental State examination, a sleep latency test, and a sleep log.
  • Monitoring occurred 1 day preoperatively and 5 days postoperatively.

Main Results:

  • Confusion, indicated by low Mini-Mental State scores, was highest on postoperative Days 1 and 2.
  • This confusion correlated with insomnia (reduced sleep time) on the night following the confusion, not the preceding night.
  • Sleep loss did not precede the onset of confusion.

Conclusions:

  • Sleep loss appears to be a consequence, rather than a cause, of postcardiotomy confusion.
  • Clinical interventions should prioritize addressing confusion directly, not solely insomnia.
