Background:

  • Congenital neck masses in children, including dermoid and thyroglossal duct cysts, require accurate preoperative diagnosis.
  • Standard ultrasound assessment has limitations in differentiating these cystic lesions.

Purpose of the Study:

  • To validate and compare the diagnostic accuracy of novel ultrasound scoring systems, the 4S algorithm and SIST score, for pediatric dermoid and thyroglossal duct cysts.

Main Methods:

  • Retrospective review of 134 pediatric patients with histopathologically confirmed thyroglossal duct or dermoid cysts.
  • Radiologists blindly assessed preoperative ultrasounds using the 4S algorithm and SIST score.
  • Statistical analysis compared diagnostic accuracy of clinical impression, standard ultrasound, and the novel scoring systems.

Main Results:

  • The 4S algorithm and SIST score achieved 84% diagnostic accuracy, significantly higher than standard ultrasound (31%) and clinical diagnosis (52%).
  • Of 134 patients, 90 had thyroglossal duct cysts and 44 had dermoid cysts.
  • No significant difference in accuracy was found between the 4S algorithm and SIST score.

Conclusions:

  • The 4S algorithm and SIST score enhance diagnostic accuracy for pediatric congenital neck masses.
  • Further research is needed to optimize preoperative assessment of these conditions.
