Background:

Synovial sarcoma is an aggressive soft tissue sarcoma, typically affecting the limbs near joints, accounting for 5-10% of all soft tissue sarcomas.
Primary involvement of the adnexa (ovary) is extremely rare, with only four cases previously described in medical literature.

Observation:

A case of a 77-year-old female patient with a rapidly growing pelvic mass is presented.
The pelvic mass was diagnosed as monophasic synovial sarcoma of the ovary.

Findings:

Synovial sarcoma originating from the adnexa represents a virtually unknown and rare disease entity.
The diagnosis of adnexal synovial sarcoma is complex and challenging.

Implications:

This case highlights the importance of considering rare diagnoses in gynecologic oncology, even when symptoms mimic more common conditions.
The poor prognosis associated with adnexal synovial sarcoma underscores the need for further research into diagnostic and therapeutic strategies for this rare malignancy.
