Background:

Odontomas are benign odontogenic tumors (hamartomas) arising from epithelial and mesenchymal components of the jaws.
They are classified into compound and complex types, with rare instances presenting combined features (compound-complex).

Observation:

A 7-year-old boy presented with a compound-complex odontoma in the right posterior mandible.
The odontoma was found to contain an unprecedented 526 individual denticles.

Findings:

Histopathological examination confirmed the diagnosis of a compound-complex odontoma.
The extreme number of denticles (526) represents a unique and clinically significant finding in the literature.

Implications:

Prompt diagnosis and surgical excision are vital to prevent complications like bony expansion and malocclusion.
Accurate histopathological assessment is essential for definitive diagnosis and management of odontomas.
Early detection and treatment of odontomas generally lead to favorable prognoses with low recurrence rates.
