Observation:

• A young male presented with chest pain post-dental procedure, later developing a masseterian phlegmon and internal jugular vein thrombosis.
• Septic embolization to the lung occurred, complicated by empyema.
• Diagnosis was delayed due to negative blood cultures.

Findings:

• The case highlights the diagnostic challenges of Lemierre's syndrome, particularly with negative blood cultures.
