Background:

Function-focused care (FFC) is crucial for managing patients with diverse cognitive statuses.
Web-based case management systems offer potential for structured FFC documentation and planning.

Purpose of the Study:

To identify functional care problems, NANDA-I nursing diagnoses, and intervention plans for patients with different cognitive statuses.
To evaluate the utility of a web-based case management system in function-focused care.

Main Methods:

Retrospective descriptive research design.
Analysis of 119 inpatient records from a nursing home in South Korea.
Utilized data from a trained web-based case management system.

Main Results:

Key physical, cognitive, and social functional problems were identified.
Nursing diagnoses were classified across six NANDA-I domains.
Intervention plans were mapped to identified functional problems and diagnoses.

Conclusions:

FFC case management information can guide interdisciplinary caregivers in implementing patient-centered interventions.
