Racial disparities and predictors of functioning in schizophrenia

Black Americans with schizophrenia show lower neurocognition and more disorganized symptoms. However, race did not impact how these factors predict functional outcomes, suggesting other factors may explain disparities in schizophrenia.
Area of Science:
- Psychiatry
- Neuroscience
- Health Disparities
Background:
- Black Americans face disproportionately higher rates of schizophrenia spectrum disorders and worse outcomes.
- Existing research on functional outcomes in Black Americans with schizophrenia is limited.
- Understanding racial disparities in schizophrenia requires examining predictors like neurocognition, social cognition, and symptom severity.
Purpose of the Study:
- To examine racial differences in functioning and key predictors (neurocognition, social cognition, symptom severity) among individuals with schizophrenia.
- To investigate if these predictors similarly influence functioning in Black and White Americans with schizophrenia.
- To identify potential factors contributing to worse functional outcomes in Black Americans with schizophrenia.
Main Methods:
- Assessed neurocognition, social cognition, symptom severity, and functioning in 66 participants with schizophrenia.
- Compared these measures between Black and White participants.
- Analyzed whether race moderates the relationship between predictors and functioning.
Main Results:
- Black participants had lower neurocognition and higher disorganized symptoms than White participants.
- No significant racial differences were found in overall functioning or social cognition.
- Race did not moderate the association between neurocognition, social cognition, symptom severity, and functioning.
Conclusions:
- Established predictors like cognition and symptoms do not fully explain functional outcome disparities in schizophrenia between Black and White individuals.
- The findings suggest a need to explore additional domains, such as structural factors and racism-related stress, to understand racial disparities in schizophrenia.
- Current psychosocial treatments focusing on cognition may require re-evaluation to effectively address racial disparities in functioning.
