To describe a novel surgical approach for managing neglected elbow dislocations with multi-directional instability.
To evaluate the functional outcomes of open reduction and circumferential ligamentous reconstruction using a gracilis tendon graft.
To highlight the potential of this technique in improving patient mobility and reducing external fixation needs.

Surgical procedure involved open reduction of the elbow joint.

Circumferential ligamentous reconstruction was performed using a gracilis tendon graft.

Main Results:

Excellent functional outcome was achieved, as assessed by the Mayo elbow performance index.
The circumferential reconstruction technique proved viable for managing neglected dislocations.
The procedure reduced the need for and duration of external fixation, facilitating early mobilization.

Conclusions:

Open reduction and circumferential ligamentous reconstruction with a gracilis tendon graft is an effective treatment for neglected elbow dislocations.
This technique offers excellent functional recovery and allows for early mobilization.
The indications for this circumferential technique can be extended to manage complex neglected elbow dislocations.
