Problems of anticoagulation with warfarin in hyperthyroidism.

H A Kellett, J S Sawers, F E Boulton

    The Quarterly Journal of Medicine
    |January 1, 1986

    Summary

Hyperthyroid patients show increased sensitivity to warfarin, requiring adjusted dosing. This study found hyperthyroidism alters clotting factors, leading to a greater anticoagulant response.

    Area of Science:

    • Endocrinology
    • Hematology
    • Pharmacology

    Background:

    • Clinical observations suggest hyperthyroid patients are more sensitive to warfarin's anticoagulant effects.
    • Warfarin's efficacy is monitored using clotting factors and coagulation times.

    Purpose of the Study:

    • To investigate the impact of hyperthyroidism on warfarin's anticoagulant effects.
    • To compare warfarin's effect on clotting factors in hyperthyroid versus euthyroid states.

    Main Methods:

    • Measured clotting factors (II, VII, VIII, IX, X), prothrombin ratio (PTR), and partial thromboplastin time with kaolin (PTT-K) in five patients.
    • Assessed these parameters and warfarin levels in both hyperthyroid and euthyroid states after a single warfarin dose.

    Main Results:

    Related Experiment Videos

    • Hyperthyroidism was linked to lower factor II activity and shorter PTT-K.
    • Warfarin caused a more significant reduction in factors II and VII and a greater increase in PTR and PTT-K in hyperthyroid patients.
    • The prothrombin ratio (PTR) showed a relatively greater increase than PTT-K in hyperthyroidism.

    Conclusions:

    • Hyperthyroidism enhances the anticoagulant response to warfarin.
    • Therapeutic anticoagulation in hyperthyroid patients may necessitate extending the PTR beyond standard ranges to achieve adequate PTT-K prolongation.
